As a member of the Lutheran Services Home Care team, you will work as part of a supportive team assisting older people in Toowoomba to remain independent and in their own home and community. **Salem** has been part of the Toowoomba community since 1935. Our **Home Care** team provides tailored care and services to our clients to support them to live independently in their own homes.

**About the role**:
As a Support Worker (Home Care Packages), you will be responsible for:

- Delivering services for clients at home and in the community as outlined in their Goal Plan including supporting and guiding clients to complete identified tasks, as well as completing agreed tasks on behalf of clients.
- Contributing to assessments of the client’s goals, interests and needs.
- Assisting the home care coordinator to maintain appropriate communication with carers and family members nominated by the client.
- Adhering to risk mitigation procedures and bring new risks to the attention of the service coordinator.
- Advising the home care coordinator of significant changes in the client’s capacity and/or if the client requests a different service mix.
- Maintaining accurate comprehensive case notes.
- Undertaking administrative duties associated with the role.

**About you**:
Critical to your success in this role will be the proven ability to demonstrate:

- A high level of effective interpersonal and communication skills
- Your ability to maintain confidentiality and impartiality
- Respect and responsiveness to client needs
- A willingness to work with non-traditional approaches to service delivery for older people

**It is essential that you**:

- Maintain a valid, unrestricted Qld Driver's licence
- Have a well maintained, clean & reliable vehicle with fully comprehensive insurance
- Are fully vaccinated for Covid 19
- Maintain a NDIS Worker Screening check
