About the Role
We are seeking an experienced and compassionate **NDIS Support Coordinator** to join our team. In this role, you will work closely with NDIS participants, their families, and service providers to ensure they receive the support and services needed to enhance their quality of life. You will assist participants in understanding and implementing their NDIS plans, connecting them with relevant supports, and ensuring they achieve their goals effectively.
Key Responsibilities
- Assist NDIS participants in understanding and implementing their NDIS plans.
- Coordinate and connect participants with appropriate service providers and community resources.
- Support participants in building their capacity to independently manage their supports.
- Monitor plan progress and ensure services align with participants’ goals.
- Maintain accurate records and documentation in line with NDIS requirements.
- Collaborate with participants, families, and other stakeholders to ensure a seamless service experience.
- Advocate for participants’ needs and support them in navigating challenges within the NDIS framework.
- Ensure compliance with all NDIS policies and guidelines.
About You
To be successful in this role, you will have:
- A strong understanding of the NDIS and experience in support coordination or case management.
- Excellent interpersonal and communication skills to engage effectively with participants and stakeholders.
- Strong problem-solving skills and the ability to work independently.
- Ability to multitask and manage time efficiently in a fast-paced environment.
- Compassionate, professional, and dedicated approach to participant support.
Qualifications & Experience
- Relevant qualifications in Social Work, Disability, Community Services, or a related field (preferred but not essential).
- Minimum 1-2 years of experience in NDIS support coordination, case management, or a similar role.
- Experience working with people with disabilities or mental health conditions.
- Knowledge of local disability support services and community resources.
- NDIS Worker Screening Check (or willingness to obtain).
- Current Police Check and Working with Children Check.
- Valid Driver’s License and access to a reliable vehicle.

How to Apply

Pay: $45.00 - $65.00 per hour
Expected hours: 5 - 20 per week
Schedule:
- Afternoon shift
- Monday to Friday
- Morning shift
- Weekend availability
Supplementary Pay:
