**What You’ll Do**:
In this dynamic and rewarding role, you’ll:

- Partner with participants to design and implement personalised, strengths-based NDIS plans.
- Connect people with the services and supports that align with their goals and needs.
- Use recovery-oriented and person-centred approaches to build skills and confidence.
- Coordinate service delivery, monitor progress, and adapt plans as required.
- Collaborate with a multi-disciplinary team and contribute to service innovation.
- Maintain accurate records and ensure alignment with NDIS practice standards.

**What You’ll Bring**:

- Significant experience in the disability or human services sector.
- Knowledge of intervention models and ability to coordinate services.
- Specialist knowledge of NDIS and related support services.
- Strong problem-solving and conflict-resolution skills.
- Ability to advocate effectively for participants and promote their independence.

**Your Toolkit**:

- Tertiary qualification in community services, social work or equivalent and extensive community work and training (essential)
- NDIS Orientation Module (essential)
- Current SA driver’s licence (essential)
- Current Safe Environments for Children and Young People - Through Their Eyes Certificate (essential)
- Current DHS Working with Children Check (essential)
- Current DHS NDIS Worker Screening Check (essential)
