**About the role**
Assist NDIS participants to coordinate and implement a range of support services, in line with their NDIS plan, using a person centred active support approach. You will establish strong rapport and relationships with a range of stakeholders including People with Disability and/or mental health issues, their family members, NDIS Local Area Coordinators, NDIS Planners, internal and external service providers.
This role is a 12 month secondment opportunity that can be either 3 or 4 days per week where you will have the flexibility to work from home and/or from our Unley office. There will be some travel required to meet with clients across the Adelaide region so an SA driver’s licence and your own reliable vehicle is required.
**Key responsibilities**
- Strengthen and enhance our client's ability to coordinate supports and participate in the community
- Regularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budget
- Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
- Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency
**Skills & experience**
- Demonstrated experience in working with people with a disability and/or mental health issues
- The ability to work in a KPI driven environment where motivation and commitment are essential
- Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDIS
- Strong written and computer skills for the creation of plans, programs reports, correspondence and other documents
- Ability to work autonomously and use initiative
