About the Role:
As an NDIS Care Coordinator, you'll play a key role in supporting individuals through their NDIS journey. You’ll be responsible for coordinating personalised care plans, advocating for participants, and collaborating with families and providers to ensure positive outcomes.What We’re Looking For:

- Experience in NDIS support coordination or a related care field
- Strong interpersonal and problem-solving skills
- Relevant qualifications in disability, social work, or case management
- A client-first approach and a commitment to professional integrity

**Location**: Bacchus Marsh and Truganina
**Employment Type**: Full-time

Pay: $27.73 - $35.76 per hour

Expected hours: 37.5 per week
