**About The Role**

**Our Care Team is the heart of our business and has a genuine passion and interest in enabling our consumers to remain living independently. We have an exciting opportunity for like-minded Case Managers (known internally as Care Partners), who will be responsible for managing a personal portfolio of clients from our office in Fortitude Valley.**

**We are seeking multiple, full-time applicants with customer service or case management experience who will champion our consumer-directed care approach, and provide ongoing support to consumers.**

**Experience in a health or aged care setting is desirable, but not necessary.**

**Duties**
- **Become the trusted point of contact for all aspects of the consumer's Home Care Package, ensuring ongoing communication with clients and/or their representatives, stakeholders, and service providers**:

- **Assist consumers to enact their care plans and budgets, ensuring needs and goals are supported under the Home Care Package framework.**:

- **Source & manage information to support consumers in achieving their documented care outcomes.**:

- **Update and maintain consumer records for all matters related to care management as required.**:

- **Identify concerns in consumer health and wellbeing for escalation**

**Salary & Benefits**
- **$60,000 - $65,000 per annum (commensurate with experience) + 10.5% super + performance bonuses**:

**Mandatory Requirements**
- **Current Federal Police Clearance (not more than 12 months old) and international Police Check if you have lived overseas in the last 10 years**:

- **Evidence of Australian work rights (Passport, Birth Certificate, etc).
