About the Role
- To provide a comprehensive psychosocial intervention (under direction of a treating Clinician) and community linkages to support mental health consumers to access a range of services to promote recovery.
- To assist and support the Transitional Housing Team in the implementation of consumer care.
- Participate in a network of referral and support through consulting and liaising with other health care professionals and agencies involved with consumer care and case management and promote and maintain community awareness of Mental Health issues.
- Utilise relevant information systems as directed for the purpose of recording and tracking clinical documentation.
