**Position Overview**

We are looking for dedicated In-Home Carers to provide high-quality, person-centered. Your role will be crucial in helping people achieve their goals, enhance their quality of life, and improve their physical and mental health.

**Key Responsibilities**

As an In-Home Support Worker your responsibilities will include:

- **Individualised** Support: Providing a range of services tailored to meet the unique needs of clients, such as housekeeping, personal care, toileting, meal preparation, medication management, and more.
- **Knowledge and Care**: Demonstrating an understanding of health and well-being, transition plans, and behavior support plans. You will also be knowledgeable about protocols regarding BSP and medication requirements.
- **Community Inclusion**: Assisting clients in participating in community activities and services based on their preferences and abilities. You will help them establish and maintain relationships and interests.
- **Communication**: Providing regular feedback to the In-Home Care Manager regarding issues that require follow-up. This includes matters related to Participants, their families, external supports, fellow employees, and community access activities.
- **Compliance**: Adhering to UFC's Code of Conduct, policies, and Quality Management System procedures to ensure the highest standards of care and support.
- **Safety and Wellbeing**: Demonstrating a strong commitment to the safety and well-being of yourself, Participants, and others in the work environment.

**About You**

You will thrive and succeed in our team by bringing the following experience to this position:

- Preferable 2 years experience supporting people in a related sector including with a disability, mental illness, community services, aged care, or rehabilitation environment
- Drivers License
- Experience working within In-Home Care environment
- Ability to meet deadlines, work with mínimal supervision, be proactive and follow care plans.
- Effective written and verbal communication skills and a demonstrated ability to write clear case notes.
- Understanding of WHS procedures

**Desirable**:

- Qualification in community services, aged care, disability, mental health or related field
- Qualifications in nursing, assistant nursing or registered nurse
- NDIS Worker Screening Clearance (obtained from Service NSW)
- Current First Aid Certificate
- Unrestricted Australian Driver’s License
