With two main acute facilities based at Leongatha and Korumburra, GSHS offers a broad range of specialist, general, acute, subacute, ambulatory, residential aged care and community services.**About the Role**
We are seeking a suitably experienced individual to lead our Gateway Team. The Gateway program is the point of entry for all Allied Health and Community Services referrals.- The role will include, but is not limited to:
- Provide administrative support to the Primary Health Services team including intake SCTT (Service Coordination Tool Templates) completion, service specific forms and waiting list management.
- Work with Director of Primary Health to improve processes in Gateway.
- Adhere to and facilitate reporting processes for all Primary Healthcare Programs/Services.
- Manage client onboarding and reporting process as per established guidelines.
- Develop and organise monthly reporting templates for key stakeholders.
- Internal and external clinical performance reporting across different services.
- Application of healthcare guidelines to improve clinical and operational outcome.

- Current First Aid Level 2 Certificate.

**Desirable**
- Relevant qualification e.g. Certificate IV in related field or Diploma.
- Knowledge of various healthcare funding streams and eligibility criteria for public health care services
- A good understanding and awareness of the Active Service Model and Diversity

- Essential : Very good leadership skills i.e. staff management, mentoring, motivational skills and be able to design team structure that motivates team to work on common goals.
- Essential : Highly committed to maintaining; data integrity, compliance to established data management protocols and good data governance practices. Very good report writing skills.
- Essential : Able to adapt and implement changes to Gateway/Intake processes to meet Primary Healthcare Programs/Service’s needs.
- Essential : High level computer skills i.e. Microsoft Office products such as Word, Excel and PowerPoint.
- Desirable : Relevant qualification e.g. Diploma in related field or Degree
- Desirable : Knowledge of various healthcare funding streams and eligibility criteria for public health care services
- Desirable : A good understanding and awareness of the Active Service Model and Diversity
