**About the Opportunity**
We are seeking experienced, compassionate, and dedicated professionals to coordinate care services for our elderly clients. As a Care Coordinator within ECH, you will have the profound opportunity to enhance client well-being and independence while building trusting relationships with our clients and families.

Our goal is to enable our clients to live confidently and comfortably in their own homes and get more out of life. With the support and guidance of our leadership and clinical teams our Care Coordinators undertake comprehensive assessments and develop care plans for our clients that enable them to reach their individual goals.

We are looking for someone:

- Who can build rapport with clients and their families
