**About the Position**

This is a permanent, full-time position. The successful Care Coordinator will provide varying levels of coordination and transition support for our clients under the NDIS.

**Key Responsibilities**:

- Assess client needs and coordinate support programs on a case-by-case basis.
- Onboard clients in collaboration with their chosen representatives, respecting individual choices.
- Establish effective and streamlined rosters of client services based on individual preferences.
- Work collaboratively with support workers and contractors to ensure clients receive quality service in line with their program goals and expectations.
- Monitor overall program costs and ensure they are within budget parameters.
- Monitor and report on client progress.
- Maintain regular contact to build strong relationships with clients, carers, families, and other stakeholders involved.
- Plan and implement socially inclusive programs and projects for the betterment of clients.
- Maintain accurate records and documentation in accordance with organizational and legislative requirements.
- Facilitate and promote the participation of service users in community activities and events.

Pay: $75,000.00 - $80,000.00 per year

**Education**:

- Masters Degree (preferred)

**Experience**:

- Care Coordinator: 2 years (preferred)

**Language**:

- English (preferred)

Licence/Certification:

- Driver Licence (preferred)
- First Aid & CPR ,NDIS WSC and WWCC (preferred)

Work Authorisation:

- Australia (preferred)

Work Location: In person
