**This Exciting Role**

We are seeking an experienced Support Coordinator / Mental Health Recovery Coach (Psychosocial) to join our team. In this role, you will be responsible for supporting individuals with disabilities to access the services and supports outlined in their National Disability Insurance Scheme (NDIS) Plan. You will work collaboratively with participants to develop and implement recovery-focused strategies, build resilience, and connect them with a range of services that align with their individual needs across one or more providers.

**Responsibilities**
- Build and maintain knowledge of local services and organisations that customers can utilise.
- Provide recovery-oriented coaching and crisis support to participants.
- Assist in determining the budget for each support type and communicate with relevant plan managers regarding fund allocation.
- Liaise with plan managers to establish appropriate claim categories and ensure accurate fund distribution.
- Support participants in implementing their NDIS plans, with a focus on fostering independence and recovery.
- Assess and manage risk in line with organisational policies and procedures.
- Resolve issues or challenges that arise during the coordination of supports.

**About You**

To be successful in this role, you will possess the following or have a willingness to obtain:

- A minimum of a Diploma in community services, mental health, or a related field.
- Experience as a Support Coordinator and/or Mental Health Recovery Coach (Psychosocial), with a strong understanding of recovery-oriented practice.
- Ability to obtain combined disability worker screening and blue card.
- Proven ability to build strong, trust-based relationships with external service providers and participants' formal and informal supports.
- A minimum of 2 years’ experience in a relevant position

**Flexibility**

Pay: $80,000.00 - $85,000.00 per year

Schedule:

- Monday to Friday

Work Authorisation:

- Australia (preferred)

Work Location: In person

Expected Start Date: 10/02/2025
