**The opportunity**
Silverchain Multidisciplinary Care Team are looking for an experienced Social Worker to join the our Albany team on a Permanent Part Time basis working a minimum of 1 day (7.6 hours) per fortnight. This role is based out of Albany service centre, between the hours 8.30am - 5.00pm with some flexibility in start and finish times. **We are also offering the option of a Telehealth position based from our Osborne Park office in Perth.**

**Key responsibilities**
You will work closely with clients, their families and relevant others to take a holistic approach to the social and health needs of clients with an ultimate goal of reducing the number of unplanned admissions to hospitals by successfully integrating the care needs of each client.

**About you**
To be considered for this role, you must meet the following criteria:

- Tertiary qualification in Social Work and eligible membership of AASW (Australian Association of Social Workers)
- The ability to work within a multidisciplinary team
- Experience in client assessments and care plans
- Excellent communication and interpersonal skills
- High computer literacy
