**About the Role**
Research indicates that income replacement schemes do not adequately prepare individuals to transition out with the right skills, connections and supports to ensure they can lead meaningful lives, including the removal of barriers to employment.
Your energy will be invested in developing a new way of working that removes barriers through a person centric service to improve participants employability. Delivering targeted counselling and information, facilitating, and coordinating access to supports that could include health and wellbeing, Commonwealth supports, and community connections all leading to individuals being well placed to confidently pursue employment opportunities.- Deliver high quality, inclusive and flexible care coordination services including client engagement, assessment, case plan, intervention, review, and case closure.
- Collaborate with participants and their stakeholders to strengthen capacity and build resilience to be able to fully participate in the community and improve their employability.
- Promote and engage with participants and their stakeholders on the benefits of participation
- Work closely with participants, their families and health/community services to oversee and facilitate appropriate engagement, assessment, and receipt of health, community, and welfare support
- Identify employment goals and aspirations, build skills and job-readiness, and support them through interviews and into employment or lead-in employment activities
- Connect and collaborate with Employment providers to facilitate ‘warm’ transition to employment support.
- Identify needs and empower individuals to build community connectivity
- Contribute to program establishment and evolution to position program as a viable ongoing concern.
- Work closely with DXC claims managers to ensure smooth handovers of support and consistent receipt of benefits (where appropriate).
**Required Expertise**
- Tertiary or relevant qualifications in health/social services
- Demonstrated coordination/case management experience in disability, mental health, personal injury, welfare or like sectors
- Demonstrated ability to work autonomously with high degree of initiative
- Excellent communication and organisational skills.
- Understanding of challenges for individuals re-engaging with employment rkets
- Well-developed decision making, negotiation and influencing skills
