Emerald Care Services is hiring a Full time Care Coordinator role in Melbourne, VIC.

TheNDIS Client Care Coordinatorplays a critical frontline role in coordinating and managing the delivery of tailored supports to NDIS participants. The position ensures participants receive the right supports at the right time, in line with their NDIS plans and personal goals.

A key part of this role includes conducting regular participant rounds—visiting clients personally to build rapport, understand their needs firsthand, and monitor their well-being and satisfaction with services.

Responsibilities:

• Conduct participant intake assessments and develop tailored support plans.
• Coordinate and schedule services in line with participant goals and NDIS funding.
• Manage budgets and track NDIS funding usage effectively.
• Build strong relationships with participants and families through regular visits and communication.
• Advocate for participants and provide education about NDIS supports.
• Maintain accurate, compliant documentation and assist with audits.
• Collaborate with external providers and manage participant transitions.
• Identify, report, and support risk management and safeguarding.
• Support business growth through community engagement and marketing activities.
• Monitor participant progress and prepare reports for plan reviews.

Qualifications:

• Experience working with NDIS participants or similar disability support roles.
• Strong knowledge of NDIS plans, funding, and service coordination.
• Excellent communication and relationship-building skills.
• Ability to manage budgets and track funding effectively.
• Strong organizational skills with attention to detail and compliance.
• Ability to advocate for participants and work collaboratively with multiple stakeholders.
• Relevant qualifications in social work, community services, or a related field preferred.
• 2-3 years of relevant work experience required for this role

Offered Salary: $60,000 – $65,000 AUD per year (depending on experience and qualifications)
