
Care Coordinator

The Client Care Coordinator plays a critical role in ensuring high-quality care and support services for NDIS participants. This position is responsible for overseeing client intake, on boarding, collaboration and ongoing client relationship management. The Contract Administrator acts as the main point of contact for clients, ensuring their needs are met, concerns are resolved promptly, and services are delivered in accordance with NDIS guidelines.
**Key Responsibilities**:
**1. Client Intake and On boarding**:
- Manage the intake process for new clients, including initial assessments and eligibility verification.
- Collect and maintain comprehensive client information, ensuring all documentation meets NDIS compliance requirements.
**2. Service Coordination**:
- Coordinate with stakeholders in ensuring care plans are updated to reflect changes in client needs and circumstances.
- Coordinate and liaise with service providers, allied health professionals, and support coordinators to implement care plans effectively.
- Ensure timely and appropriate delivery of services as outlined in the care plan.
**3. Client Relationship Management**:
- Act as the primary point of contact for client inquiries and concerns.
- Address and resolve client complaints in a timely and professional manner.
- Conduct regular check-ins with clients to assess service satisfaction and evolving needs.
**4. Compliance and Reporting**:
- Maintain accurate and up-to-date client records in accordance with NDIS and company guidelines.
- Ensure all care plans and service agreements comply with NDIS requirements.
- Prepare and submit reports on client outcomes, service delivery, and feedback.
- Participate in internal and external audits as required.
**5. Team Leadership and Collaboration**:
- Provide guidance and support to care coordinators and intake officers.
- Collaborate with other departments to ensure seamless service delivery.
- Conduct training sessions for staff on NDIS guidelines, client engagement, and care planning best practices.
**6. Continuous Improvement**:
- Identify opportunities to improve client care processes and service delivery.
- Gather and analyze client feedback to inform service improvements.
- Stay up-to-date with changes in NDIS policies and industry best practices.
**7. Care Planning and Service Coordination**:
- Develop individualized care plans that align with client goals, preferences, and NDIS requirements.
- Regularly review and update care plans to reflect changes in client needs and circumstances.

Pay: $60,000.00 - $65,000.00 per year
Schedule:
- 8 hour shift
