About the Role
- Engage with Aboriginal and Torres Strait Islander patients and families to improve their experience, knowledge, and Palliative Care related outcomes.
- Deliver evidence based and culturally appropriate, individual Palliative Care Health Worker services including, education and support to Aboriginal and Torres Strait Islander patients and their families.
- To support patients and their families when important matters are discussed and advocate where assistance is required to improve communication (including communicating with health care professionals, government agencies and other staff).
- Participate and assist in developing (as required), communication and engagement strategies with key stakeholders to identify priority issues relating to Palliative Care that can be incorporated in annual action plans for target populations.
