**About The Opportunity**:
As the face of Care with Quality, the Care Coordinator role focuses on delivering outstanding person-centred support to our clients and care worker team. Through building meaningful relationships, this role is responsible for ensuring that clients receive the care and support they need to live the life that they choose. We are seeking a full-time Client Coordinator to be based out of our Essendon Office. Key responsibilities include:

- Matching field staff with clients, ensuring services are scheduled in line with client's needs
- First point of call for clients, Support Coordinators, families, other nominated representatives and healthcare professionals
- Ensuring all documentation for clients is accurately maintained
- Working closely with field staff to ensure scheduling is appropriate and act as first point of contact for any issues or queries
- Working with the broader team on quality and continuous improvement initiatives.
- This role requires the successful applicant to have or obtain a national police and NDIS Screening Check._

**About You**:
You are a supportive, friendly, and organised individual with excellent time management, administration, and customer service skills. You have a genuine desire to provide the best quality service to all stakeholders and are comfortable dealing with a range of priorities. Your information technology skills are evolving, and you are comfortable adapting to new systems and processes.

Practically speaking, you have rostering experience and ideally, knowledge and understanding of the aged, disability/community care sector. You have demonstrated experience in providing service delivery to clients, customers and other relevant stakeholders. Above all, you enjoy being challenged and enjoy being part of a collaborative working culture.
