**Provider Service** is looking for a Support Coordinator to join our incredible team in Sunshine, Victoria.

PROVIDER SERVICE is a registered NDIS Service Provider that provides support services for people with disabilities in Melbourne and all Suburbs, states, and territories except Western Australia. Provider Service is committed to helping people with disabilities achieve their goals and lead happy lives. We provide a wide range of services and supports as an authorized NDIS service provider, all of which are catered to the need of each participant.

Due to overwhelming large volume of caseload and growth, we arecurrently recruiting Support Coordinator position based at our head office, as a Support Coordinator, you will be required to undertake the following tasks and duties:

- **Provide Level 1: Support Connection**
- **Provide Level 2: Coordination of Supports**
- **Provide Level 3: Specialist Support Coordination**
- Caseload management
- High-level knowledge and skills in coordination of support to assist people with disability in their community or in their home.
- Assist participants to connect to and engage with, informal and mainstream funded.
- Understand the different levels of Support Coordination.
- Work within the funding levels associated with each client (track and monitor billable hours)
- Build capacity in participants to maintain supportive relationships to reduce customers’ need for support coordination.
- Implement, monitor, and review the effectiveness of individual support and negotiate supports as agreed/funded and/or as appropriate.

**Required and Desirable**:
Extensive Skills and experience in the disability sector
- Excellent communication skills
- Report Writing
- Case Load Management
- Multidisciplinary Team Collaboration
- Valid Police Check
- CPR & First Aid
- NDIS Worker Screening Check
- Working With Children's Check (WWCC)
- Driver's license and own a reliable vehicle with comprehensive insurance.
- Experienced MS office suite and other IT support systems
- Working rights in Australia

**Job Type**: Part time/ Full-time

**Work Location**: In person

**Salary**: $30 - $40 per hour
