We are seeking a highly organized and empathetic Support Coordinator to join our team. As a Support Coordinator, you will be responsible for coordinating and providing personalized support to individuals with complex needs, ensuring that they receive the necessary resources and services to achieve their goals and improve their overall well-being.**Responsibilities**:_
- Assist with plan reviews by looking at progress toward goals, looking at new or revised goals, making decisions about value for money, determining whether they achieved their goals and received value for money for their plan, and troubleshooting problems with the plan’s implementation
- Develop and regularly review progress of individual plans and support strategies
- Overseeing and coordinate supports for NDIS participants
- Maintain positive working relationships with clinical and other mental health services and other government and community agencies
- Use the NDIS myplace portal to manage your NDIS account
- **Qualifications**:_
- Qualification in Mental Health, Community Work, Social Work, or another relevant human services discipline (desirable)
- Experience working in the Mental Health field (desirable)
- **Experience**:_
- Working with people with mental health issues (essential)
- Working with and implementing individual NDIS plans (desirable)
- **Skills and Attributes**:_
- Well organised, proactive and has high attention to details
- Advanced communication and interpersonal skills
- Computing skills in Microsoft Office package and using a consumer management system
- Ability to develop a strong rapport with a people from diverse backgrounds
- Advocacy skills to advise and act on behalf of members and participants on a range of social issues

Pay: $35.00 - $43.00 per hour
