**Duties and Responsibilities**
- Conduct initial assessments to identify participants needs, goals & preferences.
- Collaborate with participants and their families to develop & implement personalised NDIS support plans.
- Identify & connect participants with appropriate service providers & community resources.
- Facilitate the establishment of service agreements & ensure timely delivery of services.
- Regularly monitor participant progress & satisfaction with services.
- Conduct reviews of support plans & make necessary adjustments to ensure optimal outcomes.
- Build & maintain relationships with local service providers, community organisations & stakeholders.
- Organise community events and activities to promote social inclusion & engagement for participants.
- Advocate for participants rights & preferences in all interactions with service providers & stakeholders.
- Provide information & support to participants regarding their rights under the NDIS framework.
- Ensure compliance with NDIS Quality & Safeguards Framework & relevant legislation.
- Maintain accurate and up-to-date documentation of participant interactions, support plans & service agreements.
- Stay informed about changes in NDIS policies, procedures & best practices.
- Participate in training and professional development opportunities to enhance skills & knowledge.
**Required Qualifications**
- Certificate III or IV in Disability
- Diploma of Community Services
- Bachelor’s Degree in Social Work, Psychology, or Community Development
**Skills**
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals & families.
- Strong organisational & time management skills, with the ability to manage multiple cases effectively.
- Proficient in using technology and software for documentation, communication, & case management.
**Certifications**
- Police Check
- First Aid Certificate
- Driver’s Licence
- Work with Children Check
- NDIS Worker Screening Check

**Experience**
- Minimum of 1 to 5 years of experience in support coordination or a similar role within the disability sector.
