About the Role**

Join **Communify’s HART 4000 Immediate Housing Response (IHR) program**, providing short-term accommodation support and housing pathways for Queensland families experiencing or at imminent risk of homelessness.

As an **IHR Families Worker**, you will:

- Manage a caseload of families living in temporary accommodation
- Conduct outreach and provide transport assistance when required
- Respond to crisis and welfare needs of families in hotels
- Help families navigate the housing system and secure alternative housing
- Deliver intensive case management, including assessments, support planning, and referrals
- Provide support around safety, wellbeing, and domestic violence where relevant

This role is **full-time, fixed term until 30 June 2026**, based in **Fortitude Valley, Brisbane**, classified under **SCHCaDS Level 5**.

You’re passionate about creating housing stability for families in need and thrive in a fast-paced, community-oriented environment. You’ll bring:

- Relevant tertiary qualifications in social work, community services, or similar
- Demonstrated experience providing case management or housing support
- Excellent communication, organisational and interpersonal skills
- Ability to manage competing priorities and respond effectively in crises
- Commitment to trauma-informed, strengths-based, and client-centred practice

**Mandatory Requirements**
- Current Open Driver’s Licence
- National Police Check
- Blue Card (Working with Children Check)
- NDIS Worker Screening Check
