Reporting to: Operations Manager
Direct Reports: Support Workers
Location: Perth Metropolitan Area

Purpose of this position
The purpose of the role is to lead a team of support workers to deliver high quality,
person-centred supports. The team leader will be responsible for ensuring all possible
plan outcomes and goals are delivered and met.
Key Responsibilities and Duties
Responsibilities and duties include but not limited to:
- Management, mentoring and coaching of Support Workers. Assist in the recruitment of Support Workers, including participating in interviews
and checking references. Provide leadership to establish and maintain a culture of professionalism,
teamwork and accountability. Encourage and support innovative thinking in the planning and delivery of
supports.. Provide performance management for Support Workers, including supervision
visits and annual performance reviews. Processing, investigation and management of Support Worker related incidents,
feedback (complaints), and WHS issues. To keep and maintain records as necessary in line with organisational
requirements. Support the scheduling team to ensure that Support Workers keep their
availability up to date, and encouraging Support Workers to fill additional shifts
when needed
- Participate in the on-call roster. Other duties as required by the Operations Manager

Understanding the people we support:
- Appreciate and understand the issues experienced by people with disability and
their families;
- Act at all times as an ambassador for people with disability;. Act in a manner that provides positive role modelling for the person being
supported.
Service Delivery:
- Ensure Support Workers provide participants with real choices in all aspects
of their daily life;
- Facilitate opportunities for Support Worker skill development into all activities;
- Ensure you and your team provide opportunities for the person being
supported to develop functional skills and engage in meaningful activities;
- Ensure you and your team promote opportunities for social, community and
civic activities;
- Ensure you and your team provide opportunities for the person being
supported to experience personal development and fulfilment;
- Ensure all individual service plans are followed as directed by the
Client Liaison officer ;
- Ensure any behaviour management plans are followed diligently and with
consistency;
- Provide accurate and timely reporting to your Client Liaison officer and People &
Culture;
- Mentor Support Workers, ensuring performance targets are met and high
quality services are provided to people with disability and their families to
achieve positive outcomes.
- Ensure that Support Workers are rostered for and attend required training in
consultation with People & Culture.
Relationships and communications:
- Build strong and effective relationships and rapport with individuals, families
and Client Liaison Officers;
- Ensure Support Coordinators are involved in all communications, and
encourage families to get in touch with Coordinators regarding any issues to
do with support;
- Develop and maintain professional and functional relationships with peers and
colleagues within Directions;
- Take responsibility for sharing personal knowledge and skills across the
organisation and provide training and mentoring through buddy shifts when
required;
- Develop and maintain professional links and networks with relevant external
organisations.

- Follow all reporting and recording requirements of Directions;

resolve potential conflict.
Environment. Ensure any procurement of goods and services include consideration of the
environmental footprint;. Minimise wastage of resource materials through reduction, reuse or recycle
principles;. Report environmental issues to appropriate people.
Continuous Improvement
- Contribute to continuous quality improvement by constantly seeking out
opportunities to improve work practices and procedures;
- Assist the quality evaluation process by identifying, incorporating best practice and
providing regular reports on quality activities undertaken as required.
Occupational Health and Safety
- Contribute to the maintenance of an occupationally safe and harassment
- free
workplace;
- Ensure occupational health and safety issues are considered in all aspects of duties
performed;
- Ensure compliance with statutory requirements and company policies and
practices;
- Report all observed safety incidents and hazards and recommend improvements
where possible;
