**About this opportunity**

As the Team Leader - Client Experience, you’ll lead a passionate team dedicated to delivering high-quality, personalised care. With a focus on building extraordinary client relationships, you’ll support Care Managers and office staff to ensure seamless service delivery. You’ll also engage with the local community to promote our in-home care services, identify growth opportunities, and help shape the future of aged care in line with our strategic goals.

Based in our Briar Hill office, you’ll be part of a supportive, values-driven team with plenty of opportunities for growth and development.

**Key Responsibilities**
- Provide leadership and oversight of clinical care delivery across Home Care Package, NDIS, and private clients.
- Support and guide Care Managers in coordinating services for complex clients within a case management framework.
- Lead and develop a high-performing Client Experience Team through coaching, mentoring, and professional development.
- Monitor performance, track KPIs, and ensure accurate client data management using the Relationship Management System.
- Drive service innovation by improving care processes and implementing quality standards in line with industry requirements.
- Contribute to strategic planning, operational efficiency, and business growth by identifying improvements and leading initiatives.

**About you**
- Completed tertiary qualifications in nursing, allied health, or a related healthcare discipline.
- Proven track record of successfully leading and managing high-performing teams in a client-focused environment.
- Strong understanding of community aged care programs, including Home Care Packages (HCP), CHSP, and other funding streams.
- Demonstrated experience in delivering case management services, including care planning, client assessments, and service coordination.
