This is an opportunity to join the Primacy Care Australia team supporting NDIS individuals living with a psychosocial disability and mental health issues, participate and engage in social and community activities.
We are seeking energetic, passionate, and highly committed individuals to be part of our growing team. We currently have a several vacancies supporting participants to achieve their NDIS goals and stay engaged in their chosen community.
Our participants have various support needs, and depending on the participant, your role as a Mental Health Support Worker may involve assistance to access the community, group activities, companionship, maintaining relationships and transportation. The Mental Health Support Worker role is vital in assisting participants to achieve their NDIS plan goals and we aim to deliver high quality, person-centered services to meet the needs of our participants.

**What are the duties and responsibilities of a Mental Health Support Worker?**
- Provide support to participants in line with Primacy Care's social model of recovery.
- Empower participants to make choices in all their lifestyle activities and meet daily living needs as well as attain better social participation in their community.
- Enable participants and build their capacity
- Provide short-term focused interventions, psychoeducation, support and guidance to participants.
- Support and assist participants to achieve their goals as per their NDIS funded supports.
- Deliver high quality, person-centered services to meet the needs of participants.
- Provide weekday and weekend support as required by participants.
- Recreational activities and community access
- Support to attend appointments in the community
- Support with meal planning, grocery shopping, making healthy life choices
- Support to research and plan activities of interest
- Ability to develop relationships on all levels
- Support to complete administration tasks and engaging with government bodies
- As part of this role, you may be required to transport the participant with your own vehicle.
**What are the required skills and qualifications?**
- Available for minimum of 4 days a week
- Minimum Australian Probationary Driver’s License, fully comprehensive insurance including business cover.
- Car Registration
- Current Working with Children's Check or willing to apply.
- Current valid First Aid and CPR Certificate or willing to apply.
- NDIS Worker Screening Check or willing to apply.
- COVID-19 Training and Control Certificate or willing to obtain.
- NDIS Orientation Certificate or willing to obtain.
- Commitment to providing exceptional support, aligning with our company values and excellent communication skills
- Ability to work autonomously
- Desired: Certificate IV or degree in mental health or relevant field
- Desired: Experience in a support worker role or relevant field
