**Salem Home Care** is part of the Salem and Northridge Salem supported living communities, operated by Lutheran Services and has been serving the Toowoomba region for more than 80 years.

**About the role**:
The role of our Home Care coordinator is to support our older Australians to live confidently, safely and independently in their homes whilst maintain effective coordination of home care support, in addition you will;
- Support clients to develop and review their goal plans
- Coordinate, monitor and review the delivery of services to meet client’s individual needs
- Undertake or arrange risk assessments associated with the delivery of services
- Promote and explain the service to the potential user and families/carers involved
- Undertake administrative duties of the role including new customer sign ups and requests for upgrades

**About you**:
To be successful for this role you will have relevant experience in Home and Community Care, Case Management and/or Aged Care. You will also;
- A passion for promoting independence and self-determination by older people
- Excellent interpersonal communication
- Experience with case management and rostering software
- Maintain a positive federal police check and Australia drivers licence
