**_Employment Type: Full-Time (76 hours per fortnight)_**

**About The Role**:
In this role, you will be responsible for coordinating our clients and their assessed needs and goal plans. You will ensure that care needs are being met through high-quality service delivery.

Your tasks will include supporting clients in developing or reviewing their Client Goal/Care Plan, coordinating and monitoring the delivery of services, maintaining appropriate communication with carers and family members, and mentoring home care workers in the community.

**About You**:
To be successful for this role you will have relevant experience in Home and Community Care, Case Management and/or Aged Care. You will also;
- A passion for promoting independence and self-determination by older people
- Excellent interpersonal communication
- Experience with case management and rostering software
- Maintain a positive federal police check and Australia drivers licence
