Care Coordinator

All Aged Care is a quality provider of in-home care services. A not-for-profit organisation, with a multi-cultural focus, we pride ourselves on delivering services which exceed client's expectations, to enable clients to remain living in their own homes. We are seeking a self-motivated and dynamic Care Coordinator (CHSP) for clients living in the community. You will enjoy working as part of a dynamic multi-disciplinary team to ensure high quality services are provided whilst building strong relationships with consumers and their families. Within this role you will be required to do the coordination, care planning and monitoring to ensure client outcomes are achieved, working closely with direct care workers, providing mentoring and guidance where needed. To be successful in this role you will have: - Genuinely passionate about caring for the elderly - Be Self-motivated with a "can do" and flexible attitude - High level communication, analytical, problem solving and organisational skills - Knowledge and ability to work with people from a wide range of backgrounds and cultures - A keenness to network and promote our services - Demonstrate ability to use computer systems including MS Word and Excel You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain), open Australian Drivers Licence. An ability to speak another language will be highly regarded. You are required to be fully vaccinated - COVID19 and Influenza. **Salary**: $70,000.00 - $75,000.00 per year **Benefits**: - Professional development assistance Schedule: - Monday to Friday **Education**: - Certificate I - IV (preferred) Work Authorisation: - Australia (required)
