Key Responsibilities:

- **Personal Care Support**: Assist with personal hygiene, bathing, dressing, and grooming based on individual needs and preferences.
- **Medication Management**: Administer prescribed medications, ensuring adherence to schedules and maintaining accurate records.
- **Daily Living Support**: Help with meal preparation, feeding, housekeeping, and maintaining a safe and clean living environment.
- **Emotional Support**: Create a positive environment through active listening, companionship, and conversation to promote mental well-being.
- **Care Planning**: Work with clients, their families, and healthcare professionals to develop personalized care plans that align with their goals and abilities.
- **Mobility Assistance**: Support with transferring, positioning, and mobility aids to ensure client comfort and safety.
- **Community Integration**: Encourage social engagement and community participation, accompanying clients to appointments and recreational activities when appropriate.
- **Record Keeping**: Maintain accurate, up-to-date records of care provided, including progress notes, incident reports, ABC Charts and health changes.
- **Health Monitoring**: Observe and report any changes in the client’s health or behaviour to relevant healthcare professionals or family members.
- **Collaboration**: Work alongside multidisciplinary teams—including healthcare professionals, social workers, and families—to provide comprehensive, coordinated care.

Skills & Experience:

- **Proven Experience**: A solid background in providing disability support, with experience in meeting diverse client needs.
- **Qualifications**: Certificate IV or higher in Disability Support (or a related field) demonstrating your professional commitment and expertise.

Essential Requirements:

- NDIS Worker Orientation Module completion
- Working with Children Check
- First Aid Certificate
- National Police Check
- NDIS Worker Screening Check

Desirable Experience:

- Experience supporting individuals with complex behaviours or needs. Mental Health and Supporting people with Drug and Alcohol Addiction.
- Valid driver’s licence and access to a comprehensively insured vehicle.

Pay: $23.23 - $42.07 per hour

Expected hours: 8 - 30 per week
