**About your new role**:
As a Service Coordinator** **within our Home Health platform, you will be the primary point of contact for our customers in **Newcastle & Lake Macquarie** **with our Branch located at Warners Bay NSW**.

To be successful in this role you will enjoy taking ownership of your customer outcomes but more importantly you are committed, solutions focussed and adaptable. This role will require you to be a Change Innovator and think outside the box to drive for best overall care for your customer.

**Key accountabilities**:

- Drive customer acquisition & growth and promote positive brand recognition in the community
- Support client management including completion of initial assessments, annual reviews, care plan development & building effective relationships with the client’s families
- Manage client escalations and complaints to a high level of resolution
- Develop effective relationships with other relevant service providers including Community Support, Clinical and Allied Health services Support continuous improvement & drive a compliance focussed portfolio

**Does this sound like you?**
- Prior Case Management or Client Account Management experience
- Ability to manage multiple priories and take ownership of customer solutions
- Experience managing financial budgets & compliance standards
- Strong stakeholder engagement with a focus on building strong internal and external relationships to deliver on customer outcomes
- Strengths including time management, conflict resolution & influencing positive outcomes
- Tech savvy and able to work across various systems efficiently
- **Aged Care Industry experience is desirable not essential - we are open to someone seeking a career change but comes from a Business or Strategic background** COVID-19 vaccinated including 2 doses (Booster is optional)
