Your responsibilities will include assessing, diagnosing, and managing acute deteriorations in RACF residents, reducing emergency department presentations, and promoting hospital avoidance strategies to minimise complications. In addition, you will actively contribute to upskilling RACF nursing staff through education on common aged care conditions and the development of care pathways for recognising and responding to acute deteriorations. You will collaborate closely with GPs and other healthcare professionals to ensure continuity of care and facilitate direct hospital admissions when required, bypassing the emergency department.
