**Job Description**:
**About the role**:
Working solely on the Commonwealth Home Support Program (CHSP), your role will conduct virtual assessments to ensure our services meet the needs of our new and existing customers. This program places a strong focus on activities that support independence and take into account each person’s individual goals and choices.

You will ensure customer information is accurately entered and updated in our system as well as ensuring the data is aligned to our policies and requirements. Key to this role will be your ability to build clear, easy-to-read, and achievable care plans, ensure tasks required are achievable within the time allocated and ensure customer satisfaction and Home Care Worker safety.

You will report to the Operations Manager (Intake & Assessments) and work closely with other internal teams to ensure we deliver for our customers.

This is an initial 6-month opportunity (full-time hours Mon-Fri) with an attractive hybrid working model (office and home) in place. We can also accommodate a part-time role at 3 days per week, if you prefer.

**About you**:

- Understanding of the Commonwealth Home Support Program (CHSP) is preferred but not essential.
- Passion for Community work/Aged Care
- Previous experience using customer management systems is preferred.
- Medium computer skills using Google, Excel, and Word
- A strong, professional phone manner and time management skills
- Ability to build strong internal relationships including working within a multidisciplinary team
- High attention to detail, an ability to meet deadlines and manage multiple priorities
- Excellent ability to build rapport with customers and team members
