The PRAP Plus team pride themselves in having a positive and supportive team culture that is recognized at Launch Housing

The Private Rental Assistance Program Plus (PRAP Plus) is an exciting initiative to provide support to households accessing private rental properties that require additional practical support to establish or maintain their housing. Launch Housing has been funded to deliver the PRAP Plus program in the Northern, Bayside and Southern Melbourne areas.

PRAP Plus provides case management support to private rental tenants and residents to achieve positive client outcomes and prevent homelessness. PRAP Plus operates with a strong focus on sustaining tenancies in consideration of a vulnerable and disadvantaged client group and works to address needs and risk factors that make tenancies vulnerable.

**What you'll be doing**

Reporting to the PRAP Plus Coordinator, you will ensure clients maintain safe, appropriate and sustainable accommodation by providing support to achieve identified goals and address issues that are impacting negatively on their tenancy. Working within an outreach-based support model of service delivery, you will provide case management that includes assessment, case planning, case co-ordination and liaison.

Provide support by offering practical assistance, identify issues or barriers to tenants with the aim to empower and educate tenants to stabilise and sustain their housing to prevent homelessness
- Engage with clients to determine eligibility for organization's services or provide referrals to other services
- Ensure clients maintain safe, appropriate, and sustainable accommodation
- Work in an outreach capacity with clients assisting them to work through issues affecting or likely to affect their tenancy
- Assist with referrals and linking in to specialized support services as required

What we're looking for*

Experience in a case management role
- Self-driven and determined to achieve positive outcomes
- Tertiary qualification in Social Work, psychology or similar
- Experience in the social welfare sector, work with clients with complex needs is desirable
- Knowledge of the Residential Tenancies Act and VCAT would be an advantage
- Excellent interpersonal skills
- A valid Victorian driver’s license
