**ABOUT THE ROLE**
Prestige Inhome Care are looking for an experienced Team Leader to lead the Home Care Package (HCP) Case Management team to deliver high-quality, client-centered care. Oversee daily operations, ensure compliance with care plans and annual reviews, and drive continuous improvement through team management and procedural enhancements. Serve as the first point of escalation for any challenges within the team and ensure that resources are managed efficiently.
You will be responsible for:
- Lead and manage the Case Management team, providing direction, training, and support to ensure high performance.
- Monitor and improve team KPIs, offering coaching and feedback for continuous development.
- Ensure compliance with care plans, conduct internal audits, and provide training to maintain audit readiness.
- Oversee daily operations, managing resources efficiently and resolving complex client issues.
- Lead team-based projects, implementing process improvements and supporting change management initiatives.
If you possess the following skills, we want to hear from you:
- Previous Team Leader or people management experience
- Experience in community care/home-based care services.
- Prior Experience in Case Management and navigating complex situations.
- Exceptional customer service skills
- Excellent communication and interpersonal skills.
- Demonstrated self-awareness and self-management ability.
- Proactive and focused on continuous improvement.
