**JOB DESCRIPTION**

**Key Responsibilities**:

- Provide personal care support, including assistance with bathing, dressing, grooming, and toileting.
- Assist clients with mobility and transfers (if applicable).
- Administer medications as per care plans and under supervision.
- Assist with meal preparation, feeding, and promoting healthy eating.
- Support clients in their daily routines and activities, ensuring they feel valued and empowered.
- Encourage and assist clients in social, recreational, and community-based activities.
- Support clients in developing skills and achieving their personal goals.
- Maintain accurate records of client care and report any changes in health or behavior.
- Provide emotional support and companionship to clients to foster a sense of dignity and respect.
- Ensure a safe and supportive environment by following health and safety procedures and protocols.
- Work collaboratively with a multidisciplinary team to create and implement individualized care plans.
- Maintain confidentiality and respect the rights of clients at all times.

**Key Requirements**:

- Certificate III in Individual Support (Disability), or equivalent (preferred, but not required).
- WWVP and NDIS Screening Check Required.
- Previous experience in disability support or caregiving is desirable.
- Strong communication skills, both written and verbal.
- Compassionate, patient, and committed to enhancing the lives of individuals with disabilities.
- Valid First Aid and CPR certification (or willingness to obtain).
- A current National Police Check (or willingness to undergo a background check).
- A valid driver’s license (if required for the role).
