Easy Healthcare is a NDIS service provider based in Melbourne, Victoria. There is an exciting new opportunity for a Level 2 Support Coordinator to join our team in Melbourne.

The primary purpose of this role is to support and empower people with disabilities so they can engage/take part in community services and mainstream services and for some to achieve desired outcomes as defined in their personalised plan. Our key aim is to work with participants and their families/carers to build the capacity to manage their plans and supports independently.

**What you will do**:

- Build and maintain pro-active relationships with NDIS registered non-registered providers
- Monitor plan outcomes and expenditure
- Take a holistic approach by working effectively with the participant to coordinate, implement and maximise on identified NDIS plan goals
- Deliver clear accurate and timely rerecord-keeping or compliance purposes
- Work closely with our accounts team

**Essential Criteria**:

- A driven individual with a passion for the Healthcare Industry - in particular, working with aged care and people with disabilities
- Self-starter and a team player that is willing to make a positive difference
- Liaison and networking
- to maintain current knowledge and build contacts for client referrals
- Service provision NDIS to work with the client to better understand and implement their NDIS plan accordingly
- Financial management/data Collection-monitoring outcomes and expenditures or client plan and maintain accurate and professional client records using our database system of choice
- Contribute to ongoing development of Easy Healthcare and understand the suite of services offered
- Generate and maintain strong community links

**Skills & Experience**:

- Must have 3 years of experience in NDIS Support Coordination/Community Case Management Role
- Passionate & Enthusiastic about the NDIS and it’s participants
- Excellent written and verbal communication
- Ability to work collaboratively with anyone in our organisation

**Additional Qualities**:

- Have patience and empathy
- Have a high level of both verbal and written communication skills
- Have exceptional people skills
- Be proactive, adaptable, and able to work autonomously
- Poses a great negotiating and networking skills

**You will need**:

- 100 Points of identification.
- A valid Driver's license and comprehensively insured motor vehicle.
- Valid NDIS Worker Screening Check.
- Working With Children's Check.
- Full Australian working rights.

**Salary**: $70,000.00 - $80,000.00 per year

Schedule:

- 8 hour shift

Supplemental pay types:

- Bonus
- Commission
- Overtime pay

Ability to commute/relocate:

- Laverton North, VIC: Reliably commute or planning to relocate before starting work (required)

- Working with Children Check (required)
- Driver Licence (required)
