**Your next role**

As a Support Coordinator with 9D Care, you will assist NDIS participants to achieve their goals by providing them with resource and information, and empowering them to make well informed decisions. You will help manage and utilise their funding by connecting them with a variety of informal, mainstream and community supports, as well as funded supports.

**Key Responsibilities**
- Coordinate the day-to-day operation of NDIS Support Coordination Services.
- Contribute towards participant assessment care planning, respond to referrals and reviews in line with 9D Care’s Policies and procedures.
- Advocate internally and externally to ensure systems and services protect the rights of participants, and give them the greatest amount of choice and control over in the decision making process.
- Ensure work practices and processes are carried out according to quality guidelines, and notify the director about quality issues as soon as practicable.
- Contribute to a culture of continuous improvement that reviews indicators of participant satisfaction, and implements actions to achieve results.
- Supporting service recipients to understand their NDIS plan, goals, and objectives.

**What we need from you**
- Working knowledge of the NDIS and disability space, however, applicants with other relevant experience are also strongly encouraged to apply.
- A Min Certificate III in disability services, aged care or any relevant qualifications or bachelor’s in psychology, OT, or relevant health field.
- Experience and skills to work competently alongside people, their family, and carers in their local community to build their capacity to participate in the community and manage some or all aspects of their plan.
- Experience working with people living with a disability.
- Knowledge of the NDIS Operational Guidelines.
- Experience in navigating NDIS systems.
- An understanding of case management practices and principles.
- Well-developed communication (both oral and written) skills with the capacity to prepare reports and correspondence.
- Competence in the use of the latest computer software programs and is ability to enter, modify and extract data accurately, including the ability to track funding for NDIS plan.
- Well-developed work organisation skills, the ability to manage time effectively and prioritise tasks.
- Ability to liaise with other health and government systems and enthusiasm about advocating for participants NDIS goals.

**Essential criteria**
- Eligibility to work in Australia.
- Working with vulnerable people card.
- Double vaccination against covid 19. (Certificate required).
- Working with Children Check.
- NDIS workers screening check (required)
- Driver Licence (required)

**What’s in it for you**
- A Full-time, permanent position with an immediate start.
- Monday to Friday, 9am-5pm with no weekend work.
- Salary: $70,000.00 - $80,000.00 per year + super.
