Omni-Care is a family owned business providing home care supports to people who are elderly or who have a disability. We are looking for a compassionate and energetic Care Manager to work with our Home Care Package clients. The Home Care Package team currently consists of six case managers and administrative support who are committed to excellent client outcomes. **The Role**
We are seeking a Home Care Package Case Manager to help in our expansion of our Home Care Package team. This individual will be assisting existing clients, and onboarding and managing new recipients in Melbourne. Your case load will be mixed including level 2 to 4 package recipients with a variety of care needs. The role reports to the Home Care Package Manager.
We are looking for Care Managers who are compassionate and empathetic. You must be capable of building positive and effective partnerships with clients, their representatives, and other stakeholders including clinical experts, allied health professionals as well as support workers. You will use your skills and knowledge of care requirements to conduct assessments, and develop consumer directed care plans focused on fostering client independence, wellness and reablement. Your partnership with clients will assist them in living a life of dignity and choice.
It is essential that you are able to recognise and respond to client clinical care requirements and health and wellbeing feedback and manage client issues and outcomes.
You will work collaboratively with other team members to ensure that our program is best practice and meets the Aged Care Quality Standards, while ensuring that client service, processes and practices are client focused and aligned to company values and goals, policies and procedures and other legislative requirements.
Ideally you will have worked in the aged care sector or you will have transferable skills from case management or clinical care in other environments. Ideally you will be able to communicate effectively with people from diverse backgrounds both in a verbal and written capacity. We respect staff that understand excellent customer service and are committed to delivering it with a sense of purpose.
A current driver's license is essential to the role which comes with a company car. A police check is also mandatory. **Our vaccination policy requires a minimum of two COVID vaccinations.**

**Next Steps**
Pay: $75,000.00 - $90,000.00 per year
