About the role
The role of Service Integration Coordinator will focus on improving collaboration between government agencies, non
- government organisations and private sector providers and will assist in facilitating the delivery of a coordinated, multi-agency support response for consumers. The Service Integration Coordinator will work directly with individual teams to plan, implement and evaluate activities that develop capabilities and competencies to ensure identified consumer needs are aligned with and delivered by the appropriate clinical and/or non-clinical service provides that effects best practice standards for mental health.
About you
- Demonstrated ability to support the planning and provision of services to people with severe mental health and complex care needs.
- Demonstrated high level community engagement skills and a capacity to develop multi-agency working partnerships that contribute to improved outcomes for people with a mental illness.
- Demonstrated knowledge of current practices, philosophies and trends in the delivery of mental health services in the National and State arenas and how this impacts on those who are at risk of falling through the gaps in service delivery and not accessing appropriate clinical and community support services.
- Demonstrated high-level interpersonal and communication (oral, written, and electronic) skills, ability to consult, negotiate and liaise effectively with key stakeholders, people with a mental illness, their families and/or carers.
- Demonstrated high level management and organisational skills including advanced analytical and problem-solving skills.
- High level interpersonal, communication, consultation and negotiation skills and ability to develop effective partnerships within the Health Service and external stakeholders.
- Experience in driving and facilitating the provision of clinical education and professional development for mental health care professionals and support staff.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
