Velocity Support Services is a registered NDIS Mental Health and Disability Service provider, servicing Ipswich, Brisbane, Logan, The Gold Coast, and the Toowoomba Areas. We’re proud to provide supported independent living, short term accommodation, mental health supports, nursing supports in home and community Access.

**Job Description.**

We are looking for an experienced, motivated, and passionate **Intake and Engagement Coordinator **to join us in making a difference to peoples’ lives. This role will initially be for a 6-month contract.

The Intake & Engagement Officer is responsible for participant intake and assessment tasks, as well as support planning, monitoring, engagement, and review support. You will ensure the support being provided best meet our participants needs and goals, liaising with Support Workers, Coordinators, as required.

This role will work closely with Service Coordinators and Managers.

**Key Responsibilities**
- Intake of new referrals and participants
- Completing filing, scanning, answering phone enquiries, setting up intake appointments, importing information into management system
- Ensure participants are supported to identify, choose, and manage their own daily and lifestyle routines
- Complete regular check ins to review and update Support Plans
- Gaining feedback from participants and staff to ensure continuous improvement
- Maintain effective internal and external stakeholder relationships

You are passionate about working in the disability sector, creating programs to empower our participants to pave a way for a lifestyle where personal, professional, and social self, find scope for growth and development. You have an incredible ability to communicate and build rapport with people, with a view to listen to understand, in order to find the best support and solution.

**Skills and Experience**
- Experience in similar position managing NDIS participants intake, engagement, and care planning processes
- Cert III or IV Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar is preferred,
- A good balance between administration and customer service experience
- Strong understanding of NDIS plans, line items, schedule of support, and service agreements
- Efficiency using Microsoft Programs and able to pick up internal systems quickly
- Ability to embrace change and use your initiative to find more efficient ways of working

**Mandatory Requirements**
- NDIS Screening checks (willing to obtain one)
- Full working Rights in Australia
- Police Clearance
- Driver Licence
- First AID
- NDIS Orientation Module

**Job Types**: Fixed term, Contract
Contract length: 6 months

**Salary**: $57,180.88 - $80,000.00 per year

Schedule:

- Monday to Friday

Ability to commute/relocate:

- Ipswich, QLD: Reliably commute or planning to relocate before starting work (preferred)
