1. Provision of personal care services including intimate personal care, social support, transport and domestic cleaning services as per a schedule.

2. Ability to complete case notes, follow instructions, and maintain close contact with the Team Leader and Community staff in relation to requests for services that fall outside of the customer schedule.

3. Communicate customer feedback in a timely manner

4. Identify when a customer’s needs have changed or risks are present, and provide a timely response directly to Community staff

6. Undertake all responsibilities in a safe manner in accordance with organisational values, culture, Code of Conduct, policies and procedures.

**Essential Selection Criteria**

1. Certificate III Aged Care, Community Care or Disability Care (or equivalent)

2. Proven experience working with customers who are home care package recipients within the My Aged Care framework, or similar field

3. Basic Microsoft office suite skills

4. Evidenced strong customer focus and a demonstrated empathy for older people and families and their significant role in the lives of older people

5. Ability to work autonomously and work effectively within a multi-disciplinary team environment

6. Demonstrated understanding of diversity and the ability to work with different cultures

7. Upto date in vaccination against seasonal influenzaq and COVID

8. Ability to drive and hold a current and valid driver’s licence

**Job Types**: Part-time, Permanent, Casual

**Salary**: $20.33 - $26.30 per hour
