**Role Description**
The Care Professional role is responsible for delivering quality care and supports to older people so they can remain living at their home. You are a kind and caring person who is also practical and can get things done. You will be working in customers home to support them to live the way they want. You are approachable and love working with older people. You can read the Care Plan and make a note of what you have done with the customer each shift. You call out when something does not look right and will always think safety first. You know what care and supports you can offer to the customer and are clear with what you can’t do. Your skills and experience are right for what you want to do and you are open to more learning. The role of the Care Professional works within the purpose and values of Hazel Home Care.
**Key Responsibilities**
- Delivering care and support that’s right for your experience and level of training - such as driving customers to the doctors or doing the clothes washing
- Supporting the customer to meet their documented needs and goals - such as going for a walk around the block or doing a puzzle with the customer
- Connecting with the customer in the way that suits their personality and interests - such as reading to the customer or simply having a chat
- Helping the customer to live fully and independently in their own home
- Documenting any risks to care and supports and alerting the Care Manager
- Noting any changes in the health or wellbeing of the customer and alerting the Care Manager

- Good listener who can also get things done
- Computer literacy
- Open to more learning and training
**Requirements**:
- Police Check
- Up-to-date COVID-19 & Flu vaccinations
- Drivers license and own car to travel to visit customers (depending on the services you will deliver)
