**About the role**
Care Coordinator Post Acute Care will provide effective monitoring and review of care plans during the month of Post Acute Care funding and ongoing communication with service providers. You will liaise with Community Program Managers and teams to facilitate appropriate referral destination to ensure streamlined care for consumers.
**Key Responsibilities**
You will:
- Provide effective monitoring and review of care plans;
- Conduct risk screening of consumers as appropriate;
- Liaise with Community Program Managers and teams to facilitate appropriate; and
- Undertake an advocacy role on behalf of clients and/or carers.
**Skills and experience**
You will have:
- A relevant tertiary qualification and demonstrated experience and skills in a social science or health related area;
- Knowledge and experience in discharge planning and community-based supports;
- Knowledge of and/or experience in working with older people, people with dementia and people with disabilities;
- Experience with working within a multidisciplinary team; and
- Highly developed interpersonal skills with the ability to develop and maintain professional relationships with all levels of management, staff and the community.
