We are currently seeking Level 1-2 Support Coordinators to provide Coordination of Supports to our growing base of Participants who engage in both Face to Face and Remote Services.

We are also open to hiring on a Part Time
- and Full-Time basis.

**Responsibilities**

As a Support Coordinator, your duties include but are not limited to:

- **Stakeholder Engagement**: Build and maintain strong relationships with clients, families, service providers, and other stakeholders to ensure cohesive support.
- **Service Coordination**: Assist clients in accessing necessary services.
- **Documentation & Compliance**: Maintain accurate records, develop support plans, and ensure all documentation aligns with NDIS requirements.
- **Crisis Management & De-escalation**: Effectively manage challenging situations and provide solutions in high-pressure environments.
- **Communication**: Ensure clear, respectful communication with all parties to facilitate effective service delivery.
- **Plan Review & Management**: Assist clients in understanding, implementing, and reviewing their NDIS plans to ensure goals are met effectively.
- **Capacity Building**: Empower clients by fostering their independence and decision-making abilities within their NDIS plans.
- **Networking & Advocacy**: Advocate for clients' needs and connect them with relevant services or community resources.
- **Risk Assessment & Mitigation**: Identify potential risks in service delivery and implement strategies to address them proactively.
- **Continuous Improvement**: Regularly evaluate services and suggest improvements to align with best practices and LifeFul’s values.

**Requirements**:

- **Cert 4 or above** in Disability, Community Services, or related field. This is not essential but desired. We do also value experience and a desire to succeed
- **2 Years Experience** in NDIS support coordination or a similar role.
- Proven **experience**in **service coordination**, **stakeholder engagemen**t, and **de-escalation**.
- Strong **communication**and **organisational**skills.

**Job Types**: Full-time, Part-time

Pay: $80,000.00 - $85,000.00 per year
