**About the role**

As Transform NDIS Support continue to grow, we are looking for a **passionate and experienced**Disability Support Workers to join our team in multiple locations across greater, northern and southern suburbs of Adelaide for a full-time position.

**Key Responsibilities**
- To support our clients by implementing acquired skills to support clients with day-to-day activities.
- To assist clients in rebuilding and retraining in skills of daily living, including personal care, domestic assistance, social/community support, transport and completing support/shift notes to compliance standards.
- To perform direct care duties for individuals’ clients as specified in the personal care profile.
- To carry out designated duties/tasks as per the service duty roster and line managers instruction.
- To support clients to establish and maintain relationships and interest through access to community activities, venue and services.
- To take case notes of every shift and maintain adequate records on the client’s progress.
- To communicate in a timely manner any incidents during the shifts.

**About You**

You must have demonstrated understanding of a person-centred approach, good verbal and written communication skills, the ability to connect with others and ability to follow individual support care plans.

**Skills, Qualification and Experience**
- Have at least 1 year experience and Certificate III/ Certificate IV/diploma in Disability and/or Mental Health and/or 2 years relevant work experience to substitute the qualification.
- Current First Aid & CPR (or willingness to obtain).
- Current and valid Driver’s License & Registered Vehicle.
- Current Working with Children Check.
- Current NDIS Worker Screening Check.
- MAPA training not mandatory but desirable.
- Demonstrated understanding of a person-centred approach.
- Good verbal and written communication skills and the ability to connect with others.
- Ability to follow individual support care plans.
- Fluency in English highly desirable.

Pay: $69,000.00 - $75,000.00 per year
