
Ndis Support Coordination Assistant

**Key Responsibilities**:
- Assist in the development, implementation, and monitoring of clients' support plans.
- Collaborate with clients, their families, and service providers to ensure seamless coordination of services.
- Communicate with clients to assess their needs and ensure they are receiving the appropriate care.
- Maintain accurate and up-to-date records of client interactions, service delivery, and progress.
- Help clients navigate available services and resources, ensuring they understand their rights and options.
- Monitor service delivery and ensure that all services are provided according to the care plan.
- Provide administrative support to the support coordination team, including scheduling appointments and maintaining calendars.
- Assist in the preparation of reports, documentation, and case notes in a timely manner.
- Coordinate and follow up on service referrals as required.
- Ensure all client confidentiality and privacy standards are maintained.
**Required Skills & Qualifications**:
- Proven experience in a support coordination, administrative, or healthcare-related role (preferred but not required).
- Strong communication skills, both written and verbal.
- Excellent organisational and time-management skills.
- Ability to work collaboratively within a team and engage with a diverse range of clients.
- A compassionate, empathetic, and client-focused approach.
- High level of attention to detail and accuracy.
- Proficiency in Microsoft Office Suite, Google and basic computer skills.
- Knowledge about the NDIS (National Disability Insurance Scheme) or other relevant frameworks (for applicable regions).
- Ability to maintain confidentiality and adhere to ethical guidelines.
**Desirable Qualifications**:
- Certificate III or higher in Disability, Community Services, or related fields.
- Previous experience working with individuals with disabilities, elderly clients, or vulnerable populations.
- Knowledge of local community services and resources.
- NDIS Working with Children's Check
- Working with Children's Check
- Drivers License and a Personal Motor Vehicle
