**We are seeking a dedicated & consumer directed Care Coordinator to provide high quality support to elders continuing to live at home**

**Permanent full time**

**Monday to Friday**

**9.00am to 5.00pm (negotiable)**

**$71,136 - $76,273 p/year range ($36-$38 p/hr) depends on skills/experience **+ 11% Superannuation + Not for Profit Packaging**

**The role is based in Kilsyth but supports clients across Metropolitan Melbourne and outer suburbs**

**About the role...**

We are looking for a dedicated and consumer directed Care Coordinator to provide high quality support to elders in the community by coordinating all aspects of enquiry management and Care coordination.

MiCare delivers a range of aged services for Elders particularly those from Culturally and Linguistically Diverse (CALD) Backgrounds. Consumer directed care underpins our home care service delivery philosophy.

The successful incumbent will strive to enhance an Elder’s quality of life as they continue to live at home, promoting independence, community participation, all through their relationship with MiCare. Some of the key responsibilities of the position include:

- Provide high quality customer service
- Respond promptly and appropriately to incoming enquiries
- Liaise with Intake team for on-boarding of new Elders
- Refer on to internal and/or external programs to address query
- Foster, develop and sustain community links through participation in MiCare’s public relations activities.
- Support execution of care plans in accordance with government guidelines
- Provide support and advocacy for Elders and their carers as required
- Be actively involved with quality review for the programs ensuring document and processes meet compliance
- Develop and maintain working relationships with relevant agencies and networks

**Qualifications /Registrations**- Diploma in Community Services, or higher (or equivalent workplace experience)

**Skills & Experience**- Strong knowledge of community aged care
- Knowledge of the working processes in My Aged Care, Data Exchange portal or Veterans Home Care (VHC) portal
- Working understanding of government guidelines for different government funded programs (e.g., Home Care Package, Commonwealth Home Support Program, VHC)
- Demonstrated knowledge of Microsoft suite of programs including high level experience in Excel, Word and Outlook and rostering
- Knowledge of the Aged Care Quality Standards and compliance
- Excellent customer service and time management skills
- High level administration skills, including data entry, and computer operations
- Understanding of and empathy with the needs of CALD communities
- Ability to speak another language other than English desirable

**A police check with no disclosable outcome is required by MiCare for this role.**

**As per Government directions, current medicare evidence of influenza and full COVID-19 **vaccination, including dose 3 are required by MiCare for this position.**
