The Patient Navigator Coordination Program is a two-year project that aims to identify the enablers and barriers to integrating renal Patient Navigators into the Australian healthcare system. Previous work has shown the importance of, need for, and desire for Patient Navigators in the complex Australian renal health systems. However, how best these Navigators work with, integrate into, and coordinate within the mainstream health system is unknown. This project therefore seeks to coordinate the teams of Patient Navigators who are helping to guide Aboriginal and Torres Strait Islander patients through renal health care - towards dialysis and transplantation - across four key sites throughout the backbone of Australia. As kidney transplantation for patients in South Australia and the Northern Territory usually occurs at the Royal Adelaide Hospital, this project therefore seeks to coordinate the work of Navigators from Darwin, Alice Springs, Port Augusta, and Adelaide.

**ABOUT THE ROLE**

Key responsibilities include:

- Supporting the CIA and CIs to deliver the objectives of the project by developing a detailed research work plan;
- Managing the coordination of Health Journey Mapping tool workshops, including initial tool design and tool implementation
- Creating, implementing, monitoring, and reporting on key project milestones, goals, workstreams, and activities, in collaboration with the CIA;
- Coordinating project team meetings;
- Supporting presentations at conferences, as well as writing and submission of abstracts

**ABOUT YOU**
- Knowledge of the health system, health policy, health programs and/or clinical practice particularly with a focus on Aboriginal health and renal health.
- Experience in health systems research, including culturally safe practices and developing models of care that improve the experiences of Aboriginal and Torres Strait Islander peoples.
- Demonstrated ability to work in a culturally safe, sensitive, and accountable manner with Aboriginal and/or Torres Strait Islander individuals, communities, and stakeholders.
- Knowledge and understanding of health and social issues relating to Aboriginal and Torres Strait Islander people and their contemporary impact.
- Experience in writing reports for senior officials and lay audiences
