An exciting opportunity exists for an experienced clinician (Occupational Therapist/Social Worker/Physiotherapist/Nurse) to join the ACE (Advice, Coordination & Expertise) ED Team as an ongoing Care Coordinator in a part time or full time capacity.
The ACE ED Team is dynamic team providing transdisciplinary care coordination services to clients presenting to the Emergency Department and in the community with clients at risk of presentation. The aim of the role is to reduce presentations / admissions and optimise patient care through the provision of Care Coordination, Rapid Allied Health service and Integrated Care planning. The service operates between 7:30am and 9:00pm, 7 days a week and we are seeking an experienced Therapist to join the team.
**To be successful in this role you will have**:
- A commitment to developing integrated services for clients with multiple and complex service needs who often ‘fall between the gaps’
- An appropriate tertiary qualification and eligible for discipline specific registration
- A minimum of 2 years clinical experience in a relevant healthcare setting including acute, subacute and/or community based care
- Experience in assessment of people with complex needs, discharge planning, case conference and care planning
- Experience working in multidisciplinary teams
- A willingness to learn and grow in the Emergency Department environment
- A passion for providing Best Care to the Western Health community
