**Qualifications/Experience**

**Certificate IV in Leisure and Lifestyle**
Previous experience coordinating Lifestyle and Recreation programs in large residential aged care facilities would be highly regarded
Experience working with clients with dementia would be highly regarded
Experience coordinating a program of activities and lead these activities with your team
Experience collecting and analysing data and be able to utilize the data to create programs and activities for the residents
Assist the Diversional Therapy teams with lifestyle assessments, care plan formulation and evaluations.
Plan, implement and evaluate the lifestyle program according to residents' needs and preferences
- Material, financial and human resource management within an established program budget
Professional care practice related to the lifestyle program such as assessment, care planning, implementation and evaluation
- Contribute to effective communication systems for residents/representatives
Have a comprehensive understanding of the Aged Care Quality Standards
Identifying new and meaningful events to keep our residents engaged
Represent the organisation and build awareness of key issues in relevant, government, community, and interagency networks.
Contribute to lions havens leadership team
Promoting our Better Together model and values of Bold, Warm and Honest

**Responsibilities**

Drive & run activities internal and external to the facility.
Individually assess the social needs of each care recipient & structure programs that are appropriate to these needs.
Encourage the participation of all care recipients in the activity program
Spend time & devote effort to the socialization of individual care recipients
Make Changes to Resident Care Plans when necessary
Liaise with volunteer organisations to match appropriate volunteers/visitors to the care recipients.
Record in the care recipient’s progress notes details of any care recipient participation in activity programs.

**Salary**: $30.00 - $40.00 per hour
